Retatrutide in Cayman: phase 3 data, fake pens and a 1979 law

Retatrutide, Eli Lilly's triple agonist, produced up to 25% weight loss in its first phase 3 obesity trial but is not approved in the UK or the US. As UK regulators seize thousands of fake pens, here is what the evidence and the Cayman Islands Pharmacy Law say.

Retatrutide is the most talked-about obesity molecule that physicians in the Cayman Islands cannot prescribe. It is a single peptide that activates three receptors (GIP, GLP-1 and glucagon), and Eli Lilly's phase 3 program has now reported mean weight loss of up to 25.0% at 80 weeks78. It is also unapproved: investigational in the US and the UK, where the regulator says it has not been authorised79. That gap has produced a large online market in products sold as retatrutide, and in October 2026 the UK's medicines regulator raided an industrial unit holding more than 12,000 fake pens9. For the Cayman Islands, where the governing medicines law dates from 1979, the questions are what the evidence shows and what the law covers.

What retatrutide is: one peptide, three receptors

Lilly scientists described the molecule, then called LY3437943, in 2022 as "a novel triple glucagon, GIP, and GLP-1 receptor agonist for glycemic control and weight loss"1. It adds glucagon-receptor activity to the GIP and GLP-1 effects that tirzepatide already combines; in the first-in-human study, weight reduction after a single dose persisted up to day 431. In a 48-week phase 2 obesity trial published in 2023, the 12 mg dose produced a mean change in body weight of -24.2%, against -2.1% with placebo2. In a phase 2 trial in type 2 diabetes, the 12 mg dose lowered HbA1c by 2.02 percentage points at 24 weeks (dulaglutide: 1.41) and body weight by 16.94% at 36 weeks3. In a phase 2a study in metabolic dysfunction-associated steatotic liver disease, liver fat fell by 82.4% at 24 weeks with 12 mg, and 86% of that group reached normal liver fat4.

TRIUMPH-1: the first big phase 3 numbers

Lilly announced topline results of TRIUMPH-1 on 21 May 2026. The 80-week trial randomized 2,339 adults with obesity or overweight and at least one weight-related condition, without diabetes, to retatrutide 4 mg, 9 mg, 12 mg or placebo78. Average weight loss was 17.6%, 23.7% and 25.0% at the three doses, against 3.9% with placebo7. In 532 participants with a body-mass index of at least 35 who continued for 24 more weeks, weight loss reached up to 30% at 104 weeks78.

Tolerability is the price. Discontinuation due to adverse events was 4.1%, 6.9% and 11.3% across the three doses, against 4.9% with placebo7. Nausea affected 42.4% at 12 mg versus 14.8% with placebo, vomiting 25.3% versus 4.8%, and mild-to-moderate dysesthesia was also reported8. A post hoc analysis of phase 2 data, published in 2026, reported falls of 54.8% in high-sensitivity CRP and up to 26.9% in non-HDL cholesterol in adults with obesity without type 2 diabetes5, and a phase 2 study in type 2 diabetes found that perceived hunger and disinhibition fell with the higher doses6. Commenting on the phase 3 results, Hannah Beba told The Pharmaceutical Journal that "retatrutide remains investigational and unlicensed, and the full safety data deserve careful scrutiny once peer-reviewed"7.

A market running ahead of approval

Demand has not waited for regulators. In June 2026 Public Citizen reported that the US FDA had issued 14 warning letters since 2024 to companies selling retatrutide, that 11 of the 14 still advertised retatrutide or other unapproved peptides, and that at least 14 hospitalizations had been reported in the FDA adverse event system10. It cited a testing company that had analyzed 2,759 retatrutide samples from December 2024 to May 2026 and rated 71 of 179 vendors (39.6%) as "poor," "bad," or "fraud"10.

In the UK, the Medicines and Healthcare products Regulatory Agency (MHRA) and West Midlands Police raided an industrial unit in Birmingham on 14 October 2026 and seized over 12,000 fake retatrutide and tirzepatide pens, plus packaging for around 30,000 more products, worth approximately GBP 2 million9. Tim Duffield, head of intelligence at the MHRA's Criminal Enforcement Unit, said: "Retatrutide has not been authorised for use in the UK"9. The MHRA listed earlier seizures: 5,000 unauthorised retatrutide and tirzepatide pens in October 2025, around 2,000 doses in February 2026 and around 12,000 doses in May 20269. The Pan American Health Organization warned in March 2026 that GLP-1 products sold through unofficial channels may be falsified, unauthorized or substandard14.

What Cayman Islands law says

The law in force on medicine imports is the Pharmacy Law, 197911. It controls "pharmaceutical products having potential danger to health if misapplied" and defines a restricted pharmaceutical as one prescribed as such "which may not be dispensed or administered except under prescription"11. Under section 11, anyone who is not the Chief Medical Officer, a medical practitioner, the Chief Agricultural Officer or a pharmacist and who "imports, dispenses or otherwise deals in restricted pharmaceuticals is guilty of an offence"11. Section 16 requires the Pharmacy Board to keep the Collector of Customs informed of everyone authorised to import, and section 20 caps the penalty at a fine of $1,000 or six months in prison, or both11.

The modern regime is on the statute book but not in force. The Pharmacy Bill, 2024 would make it an offence to import, sell by wholesale, dispense, prescribe, administer or manufacture a non-approved medicine, with a penalty of twenty thousand dollars, five years in prison, or both, and would ban internet and mail-order pharmacies with no physical premises in the islands12. The official legislation website still lists the Pharmacy Act, 2024 [Act 8 of 2024] as not in force13. An unapproved medicine without an exceptional or emergency use authorization would sit squarely within that future offence; under today's law, the question is who imports and deals in it, and the maximum fine is set at 1979 levels1112.
